II
In
addition, you will need to provide us with proof that your
company is manufacturing.
Required documentation should be one
of the following:
1.) Central Excise Certificate
2.) GMP or cGMP Certification
3.) ISO certification Certificate
4.) Copy of manufacturing
certificate
5.) Certificate of inspection for
production equipment located at your
factory
Note: All Certificates/License must
bear your company's name

III
Lastly, in addition to proof of
manufacturing, you will need to send a list
of the products (dosage forms) your
company is making. Please do not
list the products that you
only distribute/trade.
In the list, you
should
include the name of the product, the
dosage form(s) it is produced in
and the active and inactive ingredients
included in each product.
